Confidentiality of Information and
Services to Children with Disabilities
The Hood-Somervell Shared Services Arrangement, which serves Glen Rose, Tolar, Three Way and Bluff Dale School Districts is located at Glen Rose ISD, 1102 Stadium Dr./P.O. Box 2129, Glen Rose, Texas 76043, telephone 254-898-3906.
Policies and procedures concerning confidentiality of student records are available for review at the above address during working hours 8:00 a.m. until 4:00 p.m., Monday through Friday. Adult students (age 18 years and over), and parents of minor children may exercise their rights to file complaints relating to or arising from the confidentiality of their records at the above address.
Individual students' records are kept at this location and may be reviewed during regular business hours by parents and eligible students by submitting a written request to Melissa Graddick, Special Education Director. Further, student eligibility folders 7 years or older will be destroyed after 1 month of this public notice. Written request not to destroy the records received from parents or adult students with disabilities will be honored. The written request should be sent to the above address.
Also, the Hood-Somervell Shared Services Arrangement will provide free, appropriate educational services to children with disabilities ages 3-21 years. Further, if a child is visually impaired or auditorially impaired, we will provide educational services from birth-21 years.
In order to receive special education services for a child with disabilities, our referral packet must be completed, assessment and evaluation procedures must be followed, procedural safeguards must be met and the child must meet eligibility criteria established by State and Federal guidelines.
If you know of any child who is suspected to be mentally, physically, emotionally or language impaired, please contact the Hood-Somervell Shared Services Arrangement for a referral. One of the staff members will be happy to set up a time to explain our child-centered process for determining eligibility and the provision of services.
